Episode 9 - Place of other complementary examinations and therapeutic principles
Take home messages:
- Doppler ultrasound is the key initial and decisional examination, sufficient to indicate treatment at the pelvic level, but it has limitations in comprehensive evaluation of pelvic reflux.
- Pelvic MRI has an excellent predictive value of laterouterine and parametral varicose veins but is limited in the detection of reflux of other internal iliac afferents. It is especially the reference examination for the extra vascular pelvic pathology.
- Selective pelvic venography is the diagnostic and therapeutic reference examination. It allows a complete mapping of pelvic venous reflux, an accurate evaluation of the venous obstruction, the identification of drainage anomalies and an embolization in the same session.
- Phlebography should be performed for a pre-therapeutic purpose, when ultrasound criteria are in favor of treatment on the pelvic floor.
- Therapeutically, surgery has no indication in pelvic varices. The treatment is exclusively endovascular. The treatment of venous compressions is of 1st endovascular intent, but may be associated in renal venous compressions with a transposition of the superior mesenteric artery.
Messages clés :
- L’écho-Doppler est l’examen initial et décisionnel clé, suffisant pour indiquer un traitement à l'étage pelvien, mais il a des limites dans l'évaluation exhaustive des reflux pelviens.
- L’IRM pelvienne a une excellente valeur prédictive des varices latéro-utérines et paramétriales, mais est limitée dans la détection des reflux des autres afférents iliaques internes. Elle est surtout l’examen de référence pour la pathologie pelvienne extra vasculaire.
- La phlébographie pelvienne sélective est l'examen de référence diagnostique et thérapeutique. Elle permet une cartographie complète des reflux veineux pelviens, une évaluation précise de l’obstruction veineuse, l’identification des anomalies de drainage et une embolisation dans la même séance.
- La phlébographie doit être réalisée dans un but pré-thérapeutique, lorsque les critères échographiques sont en faveur d’un traitement à l’étage pelvien.
- Sur le plan thérapeutique, la chirurgie n’a pas d’indication dans les varices pelviennes. Le traitement est exclusivement endovasculaire. Le traitement des compressions veineuses est de 1ère intention endovasculaire, mais peut-être associée dans les compressions veineuses rénales à une transposition de l'artère mésentérique supérieure.

Prof. Ismaïl Elalamy, FR
A Professor of Haematology and has been the Head of the Haematology Department at Tenon University Hospital, Médecine Sorbonne Université Paris, and current President of the French Society of Angiology.
